If you have level 4 tattoo you can see the specs of level 4 empowered at the Tattoos Laboratory

Short story:
each tattoo level up by a fixed amount each level and you can choose which drawback give, empowered is like 2 levels up compared to lv4 but it gives both drawbacks of lv4.
